References

Please list below names, addresses and phone numbers of two local references. These references cannot be family members or roommates. Required information is name, address, phone, relation to you.

Do you certify that the above information is true?

Are you willing to share photos of your home and family and have a home visit from a rescue volunteer?

Are you willing for Small Breed Rescue representatives to contact you and visit your home and pet from time to time by appointment after the adoption?

If you live in the Columbia, SC vicinity, are you interested in bringing the dog to free training classes on obedience and care in northeast Columbia?(You are not obligated to do so. We only want to know if you are interested.)

Would you consider giving a pet a temporary foster home or volunteering to help rescued pets in some other way? (This is for our info only. It will not affect your application.)

If you live OUTSIDE the Southeast region, how would you plan to bring the dog home? (by car? air?) Would you come to Columbia SC to pick the dog up?

By submitting this application, you agree to cooperate with the Small Breed Rescue adoption process. You acknowledge and agree that at any point in the process, we may decide that you are not a good match for the dog you are applying for and that no reason will be given. Often we have more than one highly qualified family willing to adopt the same dog. We love to adopt our dogs to new homes, but it is our responsibility alone to decide who will adopt them.
Thank you for being willing to give a home to a rescued dog.

Before emailing this, please carefully consider whether you are truly ready to adopt. We will regard your application as a serious offer to give this pet a home for 10 to 20 years or more, as long as he / she lives. Small dogs sometimes live into their upper 20's! Processing the information will take several hours of time from one or more volunteers, and we will also call your vet clinic and personal references. Adopting a pet is a major commitment and will change the life of your family in many ways.  If you are not sure you are ready for a new family member, please consider further and submit only when you are sure.  If you are ready for this commitment, we will welcome your application, as we always have many pets in our homes waiting for adoption.  If you are not ready for such a long-term responsibility, perhaps you will consider fostering a needy small dog in your home.
